£26,500 - £28,500 per annum (One Year Fixed Term contract)

The Advice Worker (Frailty and Health specialist) is a new role to Age UK Sutton delivering a 1-year pilot of a Frailty Support Service. This will provide support specifically to Sutton residents (or those with a Sutton GP) who are identified as clinically Frail and who may need longer term support to navigate the health and social care system.
The role will sit alongside the Information and Advice team but will work closely with the team supporting those being discharged from Hospital and within our Dementia and Mental Health peer support services.
